Waiver

No waiver, no riding. Your tickets will not be released until the waiver is done, and that goes for every person on your order, including any children.

Every participant completes one, every event, every year. It is a requirement of our insurance and of Workplace Health and Safety, but it is also the document that lets us look after you properly once you are on the ground at Kilkivan.

Why we ask

Every question on the form has a job to do.

Your emergency contact and your medical history are the ones that matter most. If something happens in the arena, in the yards or out on a trail, the people looking after you need to know who to call and what you are managing. A first aider who knows about your asthma, your back injury or the medication you carry can act straight away, rather than guessing.

Age is a bracket rather than a birthday, which we hope softens the blow. We need it because our obligations are different for anyone under 18, and because it matters to whoever is treating you. Nobody here enjoys giving theirs away either.

Your riding experience tells our educators where to pitch each session, so you are stretched without being put somewhere you should not be. There is no right answer and no judgement in it, and being honest about it is what makes the event work.

If you are bringing horses, the health declaration, PIC number and vehicle details let us meet the biosecurity requirements for horses travelling to and from the event, and let movements be traced if there is ever a disease scare. That protects your horse as much as it protects everyone else's.

The information you give us is used for your safety while you are with us, and for nothing else. It is not shared with any other organisation, and your data is also protected by our privacy policy.

Before you start

Set aside ten minutes and have this ready.

You will need your order number, your emergency contact details and any medical information you need to declare, including medication and the year of your last tetanus immunisation if you know it. If you are bringing horses, you will also need their breed, sex, colour, vaccination status and microchip numbers, your PIC number and your vehicle registration.

If anything changes between now and the event, complete the form again. The most recent submission is the one we hold on file.

The waiver below affects your legal rights and obligations. Please read every part of it carefully, and only sign once you fully understand and agree to its contents. If you are completing this for a participant under 18, the waiver must be read and signed by their parent or legal guardian.

Description of Activities (2): The Cowgirls Gathering events in 2026 will be held at the Kilkivan Showgrounds, Kilkivan. Details of the event as per the information/program located on our website at www.cowgirlsgathering.com.au.

RISK WARNING

I am aware that by my participation in any activities arranged by the Provider, certain risks or dangers may occur, which could include:

  • Physical, bodily or psychological injury or death.
  • Physical exertion to which I am not accustomed.
  • Failure of equipment or use of inadequate equipment.
  • There may be no or inadequate facilities for treatment or transport to treatment if I am injured.
  • The conditions in which the activities are conducted may vary without warning.
  • I may cause injury to other persons and/or other persons may cause injury to me.
  • I may be injured or die due to the negligence, breach of contract or breach of statutory duty or guarantee of the provider.

PRIVACY STATEMENT – PRIVACY ACT 1988

By completing this form you are supplying the Provider with personal information about yourself. This information is needed to ensure your safety during your time with us. The Provider is required to collect this information by our insurance company and by the department of Workplace Health and Safety.

This information you provide will not be supplied to any other organisation or used for any other purpose than that which is stated above.

(1) Provider includes the officers, employees, agents, contractors, franchisees and assigns of the Provider.

(2) Activities includes all activities and services ancillary to or associated with the named Activity, both before and after the Activity, including transportation to and from the location of the Activity whether provided by the Provider or not, briefings, inductions, training, and the provision of information in all manuals, safety guidelines and other documentation provided to or made available to the Participant with respect to the Activity, familiarisation with clothing or equipment and methods of operation of equipment and the wearing and removal of any clothing or equipment associated with the Activity. Unless otherwise specified, a reference to an Activity is a reference to a recreational service or a recreational activity as defined in relevant legislation referred to herein.

(3) Recreational services are services that consist of participation in a sporting activity or similar leisure-time pursuit; or any other activity that involves a significant degree of physical exertion or risk and is undertaken for the purposes of recreation, enjoyment or leisure.

(4) Personal injury is bodily injury and includes mental and nervous shock and death.
